If you are not satisfied with an order, you can send a complaint to your Straumann partner using the "CAD/CAM Warranty Form".
Be sure to enter the relevant order number when you fill out the form. You will find the number on the delivery note.
Print out the form and fill out all the sections. Then send a signed copy of the completed form, together with the unsatisfactory workpiece, by mail to your Straumann partner, or hand it to your Straumann sales rep.
Please note that complaints can only be processed if accompanied by the workpiece in question.
